A Forgery Affidavit form is primarily used when the payee's signature has been forged in the process of receiving payment through a Galveston County check.

In the event that a forgery is suspected, to request a re-issue of a check, please bring the following documents:

• 2 original Forgery Affidavit forms, completed and notarized
• Vaild government-issued photo ID

An investigation will be conducted and the decision to re-issue or not will be based on the findings. If the check has been approved to be re-issused, the originating department will send it to the County Treasurer's office to be disbursed.
