When requesting an authority to cremate for a Galveston County decedent please send by fax or email the Facts of Death.  If the death is not a medical examiner case, please also include a signed Death Certificate Worksheet.  The authority to cremate will be provided within one business day.  Currently there is no charge for an Authority to Cremate waiver.  Please contact the Galveston County Health District for a Cremation Permit. 
 

Request for permits and waivers for deaths that occur outside Galveston County, please contact the county in which the death occurred.
